If you are completing the paperwork from your computer, please pay close attention if the office will need more than one copy of the form turned in. This information is provided below. Some forms in PDF format may be completed on your computer, but not saved, so be sure to print off the number you need before closing the document. All other forms need to be printed off and filled out by hand. *Boundary Exception Forms must be obtained and filled out at your resident district.

¨     Battle Ground Public School New Student Registration Form for each new student.  Fill this form out completely, front and back, for each student in the family. You can skip the sections outlined in bold if it is a repeat of information.   More and more of our communication is available through the internet, so please make sure you provide us with a legible email address.

¨     Ethnicity and Race Collection form - one form per student

¨     Home Language Survey - one form per student

¨     BGSD School Health Information Form  If your student has a health concern, it should be noted on this form.  If the student has a life threatening condition such as severe allergy, seizures, asthma, heart problems, diabetes, etc, then an Emergency Action Plan must be completed prior to the first day of school.   If over the counter or prescription medications are required at school, a "Medication in School" form must be completed by the doctor.  It should be noted that students are not allowed to carry medication of any kind while at school.  Inhalers and Epipens are the exceptions if doctor's orders state that a student is allowed to carry these medications.  Please ask for the appropriate forms at the office so the mandatory paperwork is in place before the first day of school.  The student will not be allowed to start school until this paperwork is in place.

¨     Birth Certificate - to show only for those entering public school for the first time.  Your child must be 5 years old before September 1^st.

¨     Complete Certificate of Immunization - New students to the district must have this record filled out.  If currently enrolled in a BGSD school, this record will come in your student's file, HOWEVER, check the 2011/12 requirements for any updates your student may need.  Immunization requirements for 2011/12 are included in this packet.  If updates are needed, it is your responsibility to assure that information is to us before school starts. Please bring updates to Sharon Sherlock in the main office or mail them to her attention @ HomeLink, PO Box 200, Battle Ground, WA   98604.   Make sure that the update is signed by a parent.

¨      Boundary Exceptions (Application for Attendance in Non-resident District) - This form is NOT available on line because this form is completed at your resident district if you live outside the BGSD boundaries.  Remember to leave the original with your district and bring us a copy so we know that the process has been started.  You cannot sign up for classes without this form.  Districts start this process May 2nd. (1 per student)

¨     Intent to Provide Home-Based Instruction - needed for students between the ages of 8 and 18 who are not enrolled in school full time.  When you are enrolled with us, we forward a copy of this form to our district office for you.  You can list all of your homeschooled students on the same form. (3 copies)

¨     Statement of Understanding- only one signed form per family is needed (2 Copies)

¨     Request for Records ( 2 Copies)

¨     Computer Use Form: Must be completed for any student who will be using computers at school.
