2011-12 Registration Checklist for CONTINUING Families

Most continuing families will have few or no forms to submit, but to make sure.....
Please read the following carefully as it explains the continuing family paperwork:
If you are completing the paperwork from your computer, please pay close attention if the office will need more than one copy of the form turned in. This information is provided below. Some forms in PDF format may be completed on your computer, but not saved, so be sure to print off the number you need before closing the document. All other forms need to be printed off and filled out by hand. *Boundary Exception Forms must be obtained and filled out at your resident district.
- If your student was in ClassLink, PrimeLink, CoreLink, or HSD for the 2010/11 school year, you will be letting your consultant know of your intentions for next year. If you are out of district, you will give them a copy of your Boundary Exception(available in your resident district starting May 2) for the 2011/12 school year. This information will be relayed to Sharon Sherlock, the Registrar.
- Returning Creative only families (Independent Homeschoolers) will need to fill out a 2011/12 Intent to Homeschool form for their students between the ages of 8-18. You may get these in our main office. We will forward a copy of this to the Battle Ground School District Office. (3 copies)
- Boundary Exceptions (Application for Attendance in Non-resident District) This form is NOT available on line because this form is completed at your resident district if you live outside the BGSD boundaries. Remember to leave the original with your district and bring us a copy so we know that the process has been started. You cannot sign up for classes without this form. Districts start this process May 2nd. (1 per student)
- Notes from the Nurse: If your student has an Emergency Action Plan on file or takes medication during school hours, the school nurse will be sending forms home during the month of May. By state law, this paperwork needs to be in place prior to student attendance for the new school year. If your student has developed a health concern including: severe allergy, heart problems, seizures, asthma or diabetes, please notify the office staff so that the appropriate paperwork can be completed.
- Immunization updates are needed-Tetanus (Tdap) ...it is required that students in the 6th, 7th, 8th ,9th and 10th grades have a tetanus vaccine within the last 5 years (use Sept.8th, 2011 as the date to base the 5 years on). We need this information by the start of school. If updates are needed, it is your responsibility to assure that information is to us before school starts. Please bring updates to Sharon Sherlock in the main office or mail them to her attention @ HomeLink, PO Box 200, Battle Ground, WA 98604. Make sure that the update is signed by a parent.
- Fees and Fines for the 2010/11 school year, must be cleared in order to register for next year. Call Cheryl Spencer at 885-6803 if you have questions about outstanding fees and fines.
- Remember to make sure you have your Skyward Password so that you will be able to sign-up for classes on-line(Sept. 7th) If you need a password or have forgotten it, then you need to come in person to the main office and see Cheryl Spencer or Sharon Sherlock. This is not to be confused with the HomeLink website password.
